Prescriptions
forms will be mail to you upon request, must have account
with us. Some samples of materials, devices available,
contact us for more info. Please click on the contact us
button on de left. To open your account is easy, only fill
up one of our prescriptions forms and send it to us with
your information and the cast or bio-foam of your patient's
feet (for custom orthoses). Please note that the prices of
custom orthoses is higher than Ready-To-Use orthoses and a
price list will be available only to doctors, foot care
specialists and practitioners.
Here you can
print or view our forms, for correct printing is
neccesary to change margins in your printer, you can do it
by clicking in File>>>Page Setup... Write down original
numbers for setting back the margins... Just change to:
left: 0.5 Right: 0.393 Top: 0.196
Bottom: 0.277 in inches then Click OK.
For general public
Ready to use orthoses, click
on the button below for a printer friendly form, fill up all
fields and send it with check or money order, also You can
order by telephone using your credit card*, have product
code handy. If you are not sure on your shoe size, click on
USA sizes, If you are not sure about your foot necessities,
consult your doctor.
